You will have a Summary Care Record (SCR), which will be available to people providing you with care anywhere in England.
Your Summary Care Record (SCR) will contain important information about your health, such as details of any allergies, your current prescriptions and whether you have had any bad reactions to medicines.
If you want to add new information to your record, you can discuss this with your GP.
We are gradually introducing SCRs across England. NHS Rotherham are currently working with individual practices to add these records to the Summary Care Record, as a patient you have the option whether to have a SCR or not.
We will ask your permission if we need to look to look at information in your Summary Care Record. When this is not possible, for example in an emergency when you are unconscious, we will tell you later.
